A course of treatment, not a pile of sessions

A rehab centre does not sell one session, it sells a course. So a case starts here as a course of treatment with a complaint, goals, an agreed number of sessions and a package price, and it ends with a recorded discharge. Every session is filed under it.

• Sessions remaining is counted from the sessions themselves every time it is asked for — there is no stored number to fall behind the truth
• A no-show or a cancellation is recorded without spending a session from the package, because the patient received no treatment
• A course with no set number stays open-ended, and reads “not set” rather than zero
• Hold, resume, discharge and reopen, each with its reason written down

Did the patient actually improve?

Range of motion and muscle strength are measured on a joint and a movement from the clinic's own lists, so every reading is read against the published normal range, against the course's starting point, and against the other side of the body.

• Pain before and after each session, and range of motion across the whole course
• The goal drawn on the chart as a line, so reaching it is seen rather than worked out
• A “no measurable change” reading when the recent numbers stop moving — an observation for the therapist, never a verdict from the system
• None of it is stored: correcting one normal range re-reads the entire history

The treatment that happens outside the centre

Two sessions a week is two hours; the other hundred and sixty-six are the patient's. The home exercise programme is built from the clinic's library, printed on the clinic's template, and opened from a link on the patient's phone with no login.

• Sets, reps, hold time and instructions, in the words the therapist wrote
• The patient marks “I did it today”, and it reaches you without them ringing anybody
• Revising it writes a new version and keeps the old one readable, so the sheet handed over in August still makes sense in October
• The link carries no diagnosis, no assessment, no price and no balance — the exercises and nothing else

A rehab centre loses patients quietly
