Intake Questions ​

Intake questions are the things you ask every patient — allergies, chronic conditions, whether they are pregnant, how they heard about the clinic. You write each one once, and then answer it from the prescription screen, from the patient file, or let the patient answer it themselves before they arrive.

Find them under Question.

Where to Find It ​

Click an address to open that screen on your own clinic. The first click asks which clinic you are, then remembers it. Addresses holding a {id} or {token} need a specific record, so they are shown but not linked.

Step 1 — Write the Question ​

Question → Add New Question.

Answers are checked as they are saved: a required question cannot be left empty, a number must be a number and must sit inside your minimum and maximum, a dropdown answer must be one of your options, and a date must be a real date.

TIP

Prefer Dropdown or Yes / No over Short answer wherever you can. Answers you can count are worth far more later than free text.


Step 2 — Decide Who Answers It ​

So Show Online is the switch that exposes a question to patients. It makes no difference to the two internal screens, which always show everything active.

TIP

Send the appointment link the day before the visit and the patient fills in their own history — the answers are waiting on their file when they walk in.

The dialogs list questions with a badge showing how many times each has already been answered, so repeat visits are obvious. Blank answers are skipped rather than stored as empty.

WARNING

Question order is not identical on every screen — the prescription dialog and the patient-facing pages sort priority in opposite directions. Do not rely on a specific sequence; rely on clear wording.


Step 3 — Read the Answers ​

That last row matters: answers captured from the patient file or from the patient's own link do not print on a prescription. If something must appear on the sheet, answer it in the Ask Questions dialog while writing the prescription.

Each answer keeps a snapshot of the question's wording as it was on the day. Rewording a question later never rewrites history.


Editing and Deleting ​

Question → All Question lists name, question text, answer type, status, whether it is online, and priority. Search matches the name or the text; quick chips filter Active, Inactive and Show Online.

A question that has ever been answered cannot be deleted — switch it inactive instead, which stops it being asked while keeping every answer.

Changing the answer type of a question that already has answers is allowed, but the old answers keep their original form — a question switched from Short answer to Dropdown will hold values that are not in your options list.
